Eligibility criteria for enrollment in the Hospice of Hilo program
Patients will be admitted to the Hospice of Hilo program according
to the following criteria:
- The patient has a terminal diagnosis which
is likely to lead to death within six months if the disease runs
its natural course.
- The patient, family and primary physician desire palliative/comfort
hospice care.
- The patient's attending physician has been identified
to coordinate the care of the patient and agrees to work with
the hospice interdisciplinary team.
- The patient identifies a responsible
relative, friend, caregiver system or privately hired caregiver who serves
as primary care person to provide or coordinate the 24-hour a day care
of the patient. Upon admission, the patient without a 24-hour caregiver in
the home provides a written agreement for caregiver arrangements in the event
of a declining mental or physical condition. The plan will be implemented
when the hospice Interdisciplinary Team deems necessary.
- The
patient under the age of 18 has a parent or legal guardian who
will sign the “Informed Consent for Hospice Care.”
- The environment
where the hospice services are to be provided is reasonably
clean and safe, free of environmental hazards and free from verbal
or physical abusive behavior.
- The patient/family and caregiver
provide all necessary information to ensure adequate patient care.
- The
patient resides in the Hospice of Hilo service area.
