We are currently looking for compassionate, flexible, well-organized, dependable individuals able to embrace the Hospice philosophy for the following positions:
RN- On Call Per Diem Pool:
- Provides skilled nursing care to hospice patients/families.
- Responds to phone calls and provides nursing visits for night/weekend/holiday & PTO coverage.
- Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills.
- Ability to provide accurate written documentation in a timely manner.
- Must be currently licensed as a Registered Nurse in Hawaii.
- Required: A.D.N. degree. B.S.N. preferred.
- 2 years nursing experience preferred.
Medical Social Worker – Per Diem Pool:
- Conducts initial assessments & implements psychosocial counseling. Demonstrates clinical competencies.
- Collaborates & consults with interdisciplinary team regarding the plan of care.
- Supports patient’s adjustment of terminal illness & palliative medical treatment.
- Establishes the need for & provides information, coordinates resource planning & referrals to community resources.
- Maintains professional accountability & responsibility.
- Required: Master's degree in social work and an active State of Hawai‘i license. (LSW or LCSW)
- Required: Two years minimum social work experience.
- Home/health, hospice/bereavement experience preferred
**Positions require driver’s license, auto insurance & reliable transportation. ***
