Current Job Openings: Inpatient Care Center

 

Full-time Inpatient Facility Registered Nurse:

  • Provides skilled hospice nursing care to multiple patients in a facility setting as a member of the clinical team
  • Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills
  • Current RN licensure in the State of Hawai'i AND degree required; BSN degree preferred
  • 2 years experience in a hospice, hospital or nursing home setting.

Per Diem (on-call pool) Registered Nurse (RN):

  • Provides skilled hospice nursing care to multiple patients in a facility setting as a member of the clinical team.
  • Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills.
  • Must be available for shift work as needed: day/evening/weekend/holiday.
  • Current RN licensure in the State of Hawaii and degree required; BSN degree preferred.
  • 2 years experience in a hospice, hospital or nursing home setting.

Per Diem (on-call pool) Certified Nurse's Aide (CNA):

  • Requires excellent personal care and communication skill.
  • Compassionate, flexible, well-organized and dependable individual.
  • Must be available for day/evening/weekend/holiday & PTO coverage assignments as needed.
  • Current State of Hawai’i CNA certification required.
  • Minimum 2 years home health or hospice care experience.

Medical Records Coordinator:

  • Responsible for the patient medical record, including setup, filing, scanning, maintaining, break down, chart completeness and storage
  • Responsible for clinical data management, collection, entry and reporting
  • Assists the Clinical staff in obtaining required documents related to patient care. Maintains compliance and quality control of the completed work of clinical staff
  • Possess excellent communication skills; both written and verbal
  • Ability to use sound judgment in decision making and problem solving by utilizing resource materials and communication with others
  • Self-motivated and able to work independently as well as in cooperation with others
  • Excellent organizational skills with the ability to handle multiple tasks simultaneously
  • Minimum Associates degree in Office Administration and Technology or equivalent
  • Minimum one year experience within a healthcare facility or office
  • Health management information education preferred.

Clinical Educator:

  • Responsible for planning, organizing, coordinating and implementing educational activities for clinical staff.
  • Assesses the learning needs of staff with regard to the patient populations served and coordinates or delivers appropriate educational offerings.
  • Plans and directs orientation of new clinical staff and assists with planning clinical opportunities for employees.
  • Participates in developing and enhancing tools and educations programs that promote staff education and patient services.
  • Ability to train and develop employees to meet company expectations.
  • Must be very structured, organized, detailed and able to meet deadlines.
  • Current RN Hawai’i State Licensure required.
  • BSN & 2 years nursing experience preferred.
  • Leadership and educator experience preferred.
  • Knowledge of Medicare regulations and hospice standards.

Standby Registered Nurse (RN) Full-Time:

  • Provides skilled nursing to hospice patients and their families outside of normal office hours; following the plan of care and palliative guidelines.
  • Makes home visits when necessary for problems experienced by hospice patients and their care givers.
  • Performs case manager duties as needed and may on occasion process referrals, intakes and admissions.
  • Ability to exercise independent sound judgment in planning and providing quality hospice nursing care.
  • Graduate of a RN nursing curriculum.
  • BSN & 2 years nursing experience preferred.
  • Current RN Hawai’i State Licensure requiredKnowledge of Medicare regulations and hospice standards.
  • Clinical Relations Manager:

  • Manages all aspects of sales and marketing to referral sources and potential clients.
  • Promotes and markets Hospice of Hilo to targeted referral sources through sales calls, individual and group presentations, etc.
  • Researches, develops, implements and evaluates marketing plan for all company product/services offerings.
  • Responsible for establishing strong and meaningful relationships with referral sources (i.e. hospital case managers, social workers, physicians, etc.)
  • Educates and inspires referral sources about the benefits of Hospice of Hilo’s services with the overall goal being to connect those in need of the services with the services offered.
  • Represents the mission, vision and values of Hospice of Hilo with passion and enthusiasm.
  • Minimum Bachelors degree in Marketing, Business Administration or related field.
  • Minimum of two (2) years of experience in direct sales, marketing, business development and/or managing business to business relationships in the healthcare industry.
  • Knowledge of hospital/physician referral process, end of life preferred
  • Experience in pharmaceutical sales preferred.

Nurse Practitioner:

  • Under supervision and collaboration with Medical Director, provide care services to patients and families in a variety of settings within the community.
  • Educate clinical staff, patients and families on applicable advanced illness management and pain & symptom management.
  • Prepares and maintains accurate patient records, charts and documents to support sound care practices and appropriate reimbursement for services provided.
  • Develops and maintains an excellent understanding of regulations and accreditation standards.
  • Licensed State of Hawai’i Nurse Practitioner (NP) in a specialty area that permits NP to diagnose, treat and prescribe to adult ages 18 or older (APRN RX).
  • Valid State of Hawai’i RN license and a Masters in Nursing (MSN) from an accredited Nurse Practitioner Program required.
  • Hold and maintain unrestricted license for full prescribing authority based on state and federal requirements.
  • Credentialed and privileged with health plans and facilities as required to perform duties and meet agency needs.
  • Valid CPR certification required.
  • 2 years of more Supervisory/leadership experience preferred.
  • Demonstrated successful start-up of outpatient clinics or other related programs preferred.
  • HPNA membership with CHPN certification preferred.

Full-Time Registered Nurse:

  • Highly skilled nurse with ability to exercise professional judgment using excellent critical thinking and assessment skills.
  • Provides direct patient/family palliative nursing care, education & support.
  • Current RN Hawai‘i State licensure required.
  • B.S.N. & 2 years nursing experience preferred.

Full-Time CNA (Certified Nurse's Aide):

  • Ability to independently provide direct personal care services to patients in their homes under the direction of a Registered Nurse.
  • Requires excellent personal care and communication skills.
  • Compassionate, flexible, well-organized, dependable individual who is able to embrace the Hospice philosophy.
  • Current State of Hawai'i CNA certification required.
  • Minimum 2 years home health or hospice care experience.

Per Diem (on-call pool) Registered Nurse (RN):

  • Responds to phone calls and provides nursing care coverage for  day/night/weekend/holiday & PTO as needed.
  • Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills.
  • Current RN State of Hawai’i licensure required.
  • B.S.N. & 2 years nursing experience preferred.

Full-time Spiritual Counselor:

  • Highly skilled and self motivated individual with the ability to exercise independent sound judgment in decision making, planning and provision of quality spiritual care and education.
  • Provides spiritual counseling and support for Hospice of Hilo’s patients and families, in keeping with the patient’s and families’ personal desires and belief system(s).
  • Bachelors’ degree or equivalent with experience in / and a desire to provide spiritual care.
  • Adequate educational and experiential levels to meet the needs of the patients, care givers, and regulations established by state and federal laws.
  • Prefer a person with experience providing bereavement and spiritual care in a hospice type setting.

Full-time Medical Social Worker:

  • Conducts initial assessments and implements psychosocial counseling. Demonstrates clinical competencies
  • Collaborate & consults with interdisciplinary team regarding the plan of care
  • Supports patients adjustment of terminal illness & palliative medical treatment
  • Establishes the need of & provides information, coordinates resource planning & referrals to community resources
  • Maintains professional accountability & responsibility
  • Home/health, hospice/bereavement experience preferred
  • Active State of Hawai’i license (LSW or LCSW) required
  • Master’s level degree in Social Work
  • Minimum two (2) years social work experience
 

Physical Therapist (Casual Hire):

  • Provides PT services to patients as ordered in the patient’s plan of care.
  • Provides in-service education & consultation to the interdisciplinary team as requested.
  • Performs assessments/evaluations & recommends therapeutic & functional goals for the patient.
  • Instructs patient/family in the use of therapeutic appliances & devices.
  • Must be available for work on an as need basis.
  • Required: Current State of Hawaii License.
  • Minimum 2 years experience.
