Current Job Openings: Inpatient Care Center

 

Per Diem (on-call pool) Licensed Practical Nurse (LPN):

  • Provides excellent clinical care in accordance with the LPN scope of practice.
  • Must be available for day/night/weekend/holiday assignments as needed.
  • Must be currently licensed as an LPN in Hawai`i.
  • 2 years experience in a hospice, hospital or nursing home setting preferred.

 

Per Diem (on-call pool) Registered Nurse (RN):

  • Provides skilled hospice nursing care to multiple patients in a facility setting as a member of the clinical team.
  • Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills.
  • Must be available for shift work as needed: day/evening/weekend/holiday.
  • Current RN licensure in the State of Hawaii and degree required; BSN degree preferred.
  • 2 years experience in a hospice, hospital or nursing home setting.

 

Per Diem (on-call pool) Certified Nurse's Aide (CNA):

  • Requires excellent personal care and communication skill
  • Compassionate, flexible, well-organized and dependable individual
  • Must be available for day/evening/weekend/holiday & PTO coverage assignments as needed
  • Current State of Hawai’i CNA certification required
  • Minimum 2 years home health or hospice care experience.

 

 

Current Job Openings: Hospice Homecare Services

 

Registered Nurse-- Admissions (Full-time):

  • Highly skilled nurse with ability to exercise professional judgment using excellent critical thinking and assessment skills.
  • Completes admission process in patients home, hospice facility and contracted facities.
  • Works as a part of an admission team to coordinate referrals to admission.
  • Provides direct patient/family palliative nursing care, education & support.
  • Current RN Hawai‘i State licensure required.
  • B.S.N. & 2 years nursing experience preferred.

Hospice Service Coordinator (Full-time):

  • Responsible for receiving community hospice inquiries and referrals and providing hospice information, education, and other related community liaison duties related to hospice services, referrals and intake.
  • Responsible for developing and maintaining relationships with community health care professionals and agencies.
  • Process and coordinate all inquiries/referrals from community for admission to Hospice of Hilo (HOH) program at home. Promotes continuity of care by facilitating appropriate and timely referrals. 
  • Supports and assists Hospice Liaison RN with providing information regarding HOH’s program of care and services to physicians, community health care professionals and agencies.
  • Minimum of Associates Degree; preferably in Office Administration and Technology or higher.
  • Minimum of one year experience in a healthcare environment or equivalent position.
     


Hospice Service Clerk (Full-time):

  • Embraces hospice philosophy with the ability to represent the hospice program of care accurately.
  • Part of the admissions team. Strong data entry skills necessary. Provides clerical support to the admissions team and clinical processes.
  • Maintain communication with attending physicians, hospice medical director, clinical director, on-call staff, referred patients, families, agencies/healthcare professionals regarding referral/intake status.
  • Excellent organizational and communication skills, both oral and written.
  • Competent working knowledge/skills with Windows and Microsoft Office computer software.
  • Minimum of Associates Degree; preferably in Office Administration and Technology or higher.
  • Minimum of one year experience in a healthcare environment or equivalent position.

 

Full-Time CNA (Certified Nurse's Aide):

  • Ability to independently provide direct personal care services to patients in their homes under the direction of a Registered Nurse.
  • Requires excellent personal care and communication skills.
  • Compassionate, flexible, well-organized, dependable individual who is able to embrace the Hospice philosophy.
  • Current State of Hawai'i CNA certification required.
  • Minimum 2 years home health or hospice care experience.

    **Position requires driver's license, auto insurance & reliable transportation. ***

Medical Social Worker:

  • Conducts initial assessments & implements psychosocial counseling. Demonstrates clinical competencies.
  • Collaborates & consults with interdisciplinary team regarding the plan of care.
  • Supports patient’s adjustment of terminal illness & palliative medical treatment.
  • Establishes the need for & provides information, coordinates resource planning & referrals to community resources.
  • Required: Masters Degree in Social Work AND LSW or LCSW licensure in the State of Hawai`i.
  • Required: Minimum one year social work experience in healthcare setting.
  • Home/health, hospice/bereavement experience preferred.
 

Per Diem (on-call pool) Registered Nurse (RN):

  • Responds to phone calls and provides nursing care coverage for  day/night/weekend/holiday & PTO as needed
  • Requires excellent observation, critical thinking, assessment, nursing judgment & communication skills
  • Current RN State of Hawai’i licensure required
  • B.S.N. & 2 years nursing experience preferred

Physical Therapist (Casual Hire):

  • Provides PT services to patients as ordered in the patient’s plan of care.
  • Provides in-service education & consultation to the interdisciplinary team as requested.
  • Performs assessments/evaluations & recommends therapeutic & functional goals for the patient
  • Instructs patient/family in the use of therapeutic appliances & devices.
  • Must be available for work on an as need basis.
  • Required: Current State of Hawaii License.
  • Minimum 2 years experience.
